HomeMeds is a tool built in conjunction with the nonprofit, Partners in Care Foundation whose mission is to serve as a catalyst for shaping a new vision of healthcare. Sidebench partnered with Partners in Care to help develop HomeMeds which is an evidence-based application aimed at improving medication safety for older adults by identifying potential patient medication-related hazards. Through the use of HomeMeds a social service professional can complete a comprehensive in-home medication inventory, and risk assessment questionnaire. A consultant pharmacist is then able to validate and make recommendations for changes to the medication regimen to patients and prescribers.

HIPAA Compliance and Custom Software

The team at PICF had goals of storing and transferring PI and PHI records therefore governed by HIPAA and HiTech laws and regulations. This added level of complexity requires a tech partner with specific expertise to build a platform that is compliant while providing the required feature functionality to be effective. This challenge gave Sidebench the opportunity to rely on previous experience developing custom software in healthcare with an emphasis on HIPAA compliance.

Integrated Tech Partner

We started by assisting HomeMeds with technical strategies as it related to their internal tools. Shortly after we became an integral part of their team by being a thought leader and contributor to their digital transformation initiatives. We helped optimize legacy software and reimagined the infrastructure associated with the pre-existing solution. The partnership between Partners in Care and Sidebench has allowed us to continue in our pursuit to improve patient outcomes and address social determinants of health.
