Over the past month, Sidebench has been taking a deep dive into value-based care. Our previous posts include a broad look at value-based care. In this article, we’ll look at some of the companies and leaders who have developed new forms of delivering value-based care.
Value-based care can be incorporated in many ways into our healthcare system. The incorporation of value-based care in healthcare settings can be carried out from a health systems perspective, payor restructuring, off-the-shelf tools, to public policies. We wanted to highlight a few examples of companies that are leading the adoption of new delivery models of value-based care.
- Oak Street Health has implemented a national network of value-based care clinics for adults on Medicare in areas most affected by health disparities. The clinics have a community-based approach that emphasizes preventative care. Oak Street’s award-winning, novel Electronic Health Record (EHR) system powers the company’s ability to manage patient care and assess patient care quality in its value-based system. Due to their value-based model, Oak Street halved the number of patients admitted to the hospital or ED compared to Medicare standards.
- ChenMed is a value-based care clinic on a capitation payment model that focuses on coordinating care for seniors on Medicare. The physician-led primary care model allows physicians to take on a smaller number of patients, see patients monthly, and establish a strong, trusting relationship with patients. ChenMed cites a 33% decrease in hospitalizations and ER visits amongst their patients.
- ApolloMed is one of the largest value-based care companies in the US. Using AI-powered technology, ApolloMed links payors and providers by being paid a set rate through payors in a capitation model to assume the risk of patient care through consolidated independent practice associations and accountable care organizations. They service payor populations including Medicare, Medicaid, commercial, and other private insurances as well as a variety of provider settings from primary care to specialty care services.
- Carevive is an oncology-focused company using a data-driven approach to better cancer management, improved patient outcomes, and reduced avoidable care. By bringing together clinical data and real world patient reported outcomes, clinicians can personally tailor cancer care management programs and remotely monitor patients. Data from Carevive is helping researchers understand cancer delivery models, payor reimbursement, and personalized cancer therapies to develop more effective methods of cancer care.
